Preoperative
Three progressive surgical interventions were performed.
Starting L4-L5 pedicle arthrodesis due to lumbar instability due to degenerative disease.

Operation
Diagnosed with pseudoarthrosis, we perform replacement of screws with increased thickness and double bar fixation with iliac screws. It is not possible to perform L5-S1 interbody arthrodesis due to subsidence of the intervertebral disc space.
Postoperative
He progressed very satisfactorily with significant improvement in pain and progressive ambulation with stabilization of gait and without requiring help with a cane or walker.
