Clinical case

Spine surgery

A 58-year-old man comes to the clinic with severe functional limitation for ambulation with inclination and dorsolumbar postural flexion. He has a surgical history of multilevel pedicle arthrodesis from L5 to T10.

Preoperative

Three progressive surgical interventions were performed.

Starting L4-L5 pedicle arthrodesis due to lumbar instability due to degenerative disease.

required increased levels of pedicle arthrodesis due to progressive instability and functional impotence for standing. He remains in a functional situation with global bed rest, getting up only to clean himself using a walker.

Operation

Diagnosed with pseudoarthrosis, we perform replacement of screws with increased thickness and double bar fixation with iliac screws. It is not possible to perform L5-S1 interbody arthrodesis due to subsidence of the intervertebral disc space.

Postoperative

He progressed very satisfactorily with significant improvement in pain and progressive ambulation with stabilization of gait and without requiring help with a cane or walker.
