Espondilolistesis: diagnóstico y tratamiento en Madrid

Causes, symptoms and treatment of spondylolisthesis

Spondylolisthesis is a disease in which the vertebrae move over each other, up to five degrees depending on the distance they have slipped. Spondylolisthesis is the most common cause of low back pain in adolescents. In this post we tell you what the symptoms, causes and treatment of spondylolisthesis are.

What is spondylolisthesis?

Spondylolisthesis is the sliding of a vertebra on the one immediately below it., normally affecting the bones of the lower back. Spondylolisthesis can occur at both the cervical and lumbar levels, with the symptoms of lumbar spondylolisthesis being more common.

When the displacement of the vertebra occurs forward it is called anterolisthesis and when it occurs posteriorly, it is known as retrolisthesis.

The term spondylolisthesis comes from the Greek word spondylos, whose meaning is spine or vertebra, and listhesis, which means to slide.

Qué es la espondilolistesis

Degrees of spondylolisthesis

The degree of slippage of the vertebrae can be classified from I to IV. Grade 1 (0 to 25%) indicates that the 25% of a vertebra has slipped forward on its inferior. When grade II is diagnosed, the slip exists between 25 and 50%. Grade III indicates a slip of the vertebrae from 50 to 75%. Finally, grade IV spondylolisthesis represents a slippage of 100%, known as spondyloptosis.

Generally, grade III and grade IV slips may require surgical intervention.

Types of spondylolisthesis

There are different types of spondylolisthesis. Congenital spondylolisthesis It is present from birth, as a result of imperfect bone formation.

For its part, it is known as isthmic spondylolisthesis when it appears as a result of spondylosis due to degeneration of the spine.

Degenerative spondylolisthesis is the most common type of this disease as a result of the progress of aging of the bones, joints and ligaments of the spine.

What are the symptoms of spondylolisthesis

In general, grade I and II spondylolisthesis does not usually cause pain, so symptoms appear in the most severe grades. Pain in the lower back, thighs and legs are the most common symptoms of spondylolisthesis..

In most cases, the patient reports more pain when standing or walking, causing instability in walking in the most complicated cases.

Some physical differences and symptoms of spondylolisthesis manifest as an overshooting of the abdomen, with a shorter torso appearing. Thus, spondylolisthesis can lead to increased lordosis, causing kyphosis in the final stages.

Causes of spondylolisthesis

In children, spondylolisthesis appears between the fifth bone of the lower back and the first bone of the sacral area of the pelvis, due to a congenital anomaly in this area of the spine or due to acute trauma to it.

On the other hand, in adults, the most common cause of spondylolisthesis is the abnormal wear and tear of cartilage and bones, generally in people over 40-50 years of age, with more cases occurring in women than in men.

Bone diseases and bone fractures can also cause spondylolisthesis. In addition, certain sports practices such as gymnastics, weight lifting and American football cause tension in the bones of the lumbar region, which can cause the vertebrae to weaken.

Diagnosis and treatment of spondylolisthesis

When symptoms remain present for a prolonged period of time, it is important to see a specialist to diagnose the disease and determine the appropriate treatment, before, if possible, the symptoms worsen and become a severe pathology.

How is spondylolisthesis diagnosed?

Through a x-ray of the lower back You can see if there is any vertebra out of place. In addition, a computerized topography scan or an MRI can be requested, through which more detailed images are obtained that allow us to see more clearly if the vertebrae of the spine are aligned.

How is spondylolisthesis treated?

Treatment for spondylolisthesis depends on several factors, such as the age and health of the patient, the extent of the slip, and the aggravation of the symptoms present.

As a conservative treatment, the practice of physical exercise is recommended with the aim of strengthening the muscles of the spine.

In cases where pain persists, surgical intervention will be established., which offers better results by improving symptoms and quality of life for patients.

At the Institute of Advanced Spine Surgery we have a team of traumatologists experts in minimally invasive surgery with more than 10 years of experience in the medical sector. How viable is a 3rd spinal intervention? I started with scoliosis. For that reason, they were my first two surgeries, but today I have grade 3 listhesis.
I would like to know how feasible and the percentage of postoperative improvement there is.

    • Hello Eloise,

The idea is that a specialist in the vertebral colon can evaluate the current tests in order to determine whether a trait is necessary or whether simple surveillance is sufficient.

  12. Hello, I have been diagnosed with grade 1 spondylolisthesis in the last lumbar vertebra – code M51.2G. It all started because I was sitting with my right leg crossed for too long while working on the computer, and my entire right ankle and foot became numb. Suddenly, I couldn't fully lift it or stand on my right heel. Four weeks later, I've regained those ranges of motion, but I still have a feeling of loss of strength in my ankle and ball of my foot, which makes me feel unstable. My back sometimes bothers me in the lumbar region, but not in my glutes or legs. This sensation can generally be recovered in the foot with exercise. It indicates that surgery is necessary if it doesn't go away.
    Greetings

    • Hi Pablo, thanks for sharing your case.

Grade 1 spondylolisthesis, especially if associated with symptoms such as loss of strength or sensation, can affect nerve roots and cause changes like the one you describe in your foot. The partial improvement you mention after a few weeks is a good sign, as it indicates that the nerve may be in the process of healing.

However, when a feeling of weakness or instability persists, a complete neurological and functional evaluation is recommended. In many cases, recovery can continue with time and well-guided physical therapy exercises, especially if there is no radiating pain or other signs of severe compression. Surgery is not always necessary and is usually reserved for cases with progressive neurological involvement or disabling pain that does not improve with conservative treatment.

To provide you with an accurate diagnosis, our specialists must evaluate your tests. You can make an appointment by calling (+34) 91 005 39 00.
