Back pain in children

Back pain is one of the most common pathologies among the population and one of the most frequent reasons for consultations with traumatologists. During childhood and adolescence, this problem frequently occurs, especially during the growth stage.

The location of the pain can vary, affecting any area of the back, from the cervical to the lumbar region. In some cases, the pain can radiate to other areas of the body, such as the legs, shoulders or hips, making daily activities difficult for children.

Types of back pain in children

Back pain during the growth stage is very common and, fundamentally, Two types of pain can be distinguished, depending on the causes that are causing it.

  • Non-specific pain: This is the most common type and is not associated with a specific pathology. In some cases, it may be related to minor sprains or muscle contractures.
  • Specific pain: In this case, the pain appears as a consequence of another pathology. It is common for the patient to suffer from spondylolisthesis, which affects the junction between the front and back of the last vertebra. Specific pain can also be related to tumors, infections or herniated discs.

Causes of back pain in children

The causes of back pain in children are diverse and, in the case of specific pain, they are usually related to more serious pathologies.

On the other hand, non-specific pains are usually caused by external factors. One of the most common is weight overload, as occurs when children carry loads that exceed 10% of their body weight, which directly impacts the health of their back.

In some cases, pain may be associated with a family history of spinal diseases, which increases the predisposition to suffer discomfort.

In addition, a sedentary lifestyle has become more relevant as a cause in recent years. Lack of physical activity, combined with long hours in front of screens, considerably increases the risk of developing back pain during the growth stage.

Among the most frequent pathologies related to back pain in children is spondylosis, a condition characterized by a small fracture in the last lumbar vertebra, which affects the junction between its posterior and anterior parts.

Another pathology that can cause back pain in children is discitis either spondylodiscitis, an inflammation that affects the intervertebral discs, especially in children under six years of age.

Finally, some spinal cord injuries, such as spinal cord compression, can cause significant pain, especially in the cervical region. This type of condition is more common in children under eight years of age and requires personalized medical attention.

When should you worry about back pain in children?

Although back pain in children is common, there are situations in which it requires medical attention:

  • Persistent or worsening pain:If the pain lasts for several days or increases over time, it is important to consult a specialist.
  • Pain accompanied by fever: If the pain comes with fever or general symptoms, it could indicate an infection that needs treatment.
  • Difficulty moving:If your child has trouble walking or moving because of the pain, it could be a sign of a serious condition.
  • Radiating pain or numbness:If the pain radiates to other parts of the body or is accompanied by weakness, it could be related to a nerve problem.
  • Changes in sphincter control: Loss of urinary or fecal control along with back pain requires immediate attention.

Diagnosis of back pain in children

Once the patient comes to the pediatric consultation complaining of back pain, first of all, a physical examination of the patient is carried out., in which the pediatrician studies those points in which the child suffers the most pain.

Secondly, any involvement of the spinal cord is ruled out, and a neurological assessment of the patient is carried out, in which the patient's cognitive and orientation abilities are assessed.

In some cases, diagnostic imaging tests, such as x-rays or MRIs, are necessary to rule out breaks or fractures. However, Avoid subjecting growing children to radiation.

Treatment for back pain in children

Once the patient's diagnosis has been made and the causes of back pain have been studied, the appropriate treatment is prescribed. Firstly, it is common for an anti-inflammatory treatment to be prescribed, with the aim of relieving and reducing the pain that the child experiences.

Physiotherapy treatment, carried out by a children's physiotherapist, is recommended to help muscle relaxation and postural correction.

In the most severe cases, in which the patient does not experience improvement with the previous treatments, it will be advisable for the child undergoes spinal surgery, with the aim of relieving pain and ensuring correct evolution and development of the musculoskeletal system.

Prevención del dolor de espalda en niños

Prevention of back pain in children

Although in some cases back pain in children is unavoidable due to more serious pathologies, in many others it can be prevented by following certain guidelines.

One of the key factors during the growth stage is the body weight control, as this directly influences the child's development. To achieve this, a balanced diet based on plant foods and high-quality animal proteins is essential.

Furthermore, the sedentary lifestyle should be avoided by promoting regular, moderate physical activity, adapted to the needs and abilities of the child. Exercise not only improves general health, but also strengthens the muscles that support the spine.

Finally, It is essential to avoid heavy loads, such as overly loaded backpacks, which can put pressure on the back and contribute to lower back pain.

As explained, it is important for the child to acquire certain habits during his or her growth stage that help prevent back pain. Furthermore, early diagnosis and treatment is of great importance, which helps prevent future consequences.

  1. Hello, my daughter is 10 years old and since October 2022 she has had pain in her lower back, a resonance scan and a lumbar x-ray were performed in which I found nothing. She is diagnosed with mechanical lumbago, how long does this symptom usually last and what test can I perform on my daughter, since she persists with the pain.
    I remain attentive,

    Greetings.

    • Hello, ximena.
Of course, it can last a long time if the muscles are not prepared. But it is always advisable to undergo a clinical examination to make sure that the pain is coming from the spine. Sometimes it can be a reflection of other pains (such as pancreas, kidneys), etc.
      A hug.

  8. My brother began to feel a sensation at the age of 15 and since April of this year he began to feel pain in his back but he didn't notice until June. We went to the doctors and then a specialist had x-rays and MRIs, the specialist diagnosed him. and he said that he shouldn't have anything physical for which he would have to have pain, he thinks it is due to growth and/or something emotional, I sent him to go to therapy with a specialist who has prescribed medication for 1 month now. but he does not feel improvement, we want to see what another specialist thinks because when we went to other doctors who were not specialists they said that the pain could be due to some discs that are close together and that affected the nerves, the specialist said that they were normal, they also thought that it could It could be due to a blood ball that he has (a 1.0x0.6 cm hemangioma) and that could explode and leave him unable to walk, and the specialist said that if he has it, it is like a blood ball but there is no problem with that at the moment and It shouldn't cause him pain, I wanted to know what country they are from and I will apply to my brother the back pain due to growth. He will be turning 18 in 10 days, he is no longer a child, it will go away at 21 when it is supposed to. that he stops growing, his pain is so strong that sometimes he can no longer walk, he thinks it shouldn't be something emotional because before the pain he was fine, he feels pain every day and investigated and I think he has chronic pain since he has been 5 months where at no time during the day the pain goes away, I also saw that he could have had an injury from carrying something heavy, the pain remained and he couldn't stop thinking about it and it became chronic pain, I saw that the pain lasted More than 4 weeks it was already very bad, please help.
