Mareo cervicogénico: qué es, causas, síntomas y tratamiento

The best diagnosis and treatment for cervical dizziness

Some people experience dizziness related to neck problems, known as cervicogenic dizziness. This dizziness can last for hours and be confused with other conditions, so it's extremely important to see a specialist. In this article, we'll cover everything you need to know about cervical dizziness: what it is, its symptoms, its causes, and the best diagnosis and treatment options.

What is cervicogenic dizziness?

He cervicogenic dizziness, also known as dizziness of cervical origin, It is often confused with vertigo of vestibular origin. It frequently occurs in patients who have suffered a sprain or whiplash.

This type of dizziness is characterized by neck pain accompanied by balance disorders, and is often caused by a dysfunction of the cervical spine.

¿Qué es el mareo cervicogénico?

Main symptoms of cervical dizziness

The symptoms of cervical dizziness are diverse and They can last from minutes to hours.

As the name indicates, affected people experience dizziness, which tends to worsen with certain head movements, accompanied by a sensation of movement of the environment. Therefore, patients with cervicogenic dizziness have a perception of general imbalance.

Additionally, patients often experience headache of cervical origin, which manifests as intense pain in the head, with disorientation and confusing vision. On the other hand, a limitation is observed in the cervical range of motion (ROM).

In more severe cases, patients with cervicogenic dizziness may experience difficulty walking, standing, or performing any activity.

These symptoms are often confused with those of vertigo due to their similarity. Below, we'll identify how to differentiate between these potentially similar conditions.

How do you know if it is vertigo or dizziness?

Although they are often confused, vertigo and dizziness have key differences in their symptoms and causes. To distinguish which of them you are experiencing, it is useful to pay attention to the following aspects:

  • Sensation of movement: When you experience the environment rotating or moving, it is likely vertigo.
  • Duration and frequency: Vertigo can last from minutes to hours, while dizziness can be persistent and have a variable duration.
  • Associated symptoms: Vertigo is often accompanied by nausea and vomiting, while dizziness may be associated with lightheadedness, weakness, or feeling faint.

In both cases, professional help should be sought to obtain a diagnosis and discuss appropriate treatment to control and reduce symptoms.

Causes of cervical dizziness

The causes of cervicogenic dizziness can be diverse. On the one hand, this type of dizziness can arise from a neuromuscular deficit of the deep stabilizing muscles of the neck, that is, the lack of strength of the muscles that form the cervical region.

Causas del mareo cervical

It is also common to relate cervical dizziness to pathologies such as arthritis, cervical osteoarthritis, and even herniated discs. In addition, certain postural alterations can trigger this type of dizziness.

Other factors may contribute include presence of supranumerary ribs or cervical ribs, which develop at the level of the last cervical vertebra. Another less common cause is due to vascular spasms, caused in many cases by smoking and stress.

Changes in cervical dizziness

Within cervical dizziness, various alterations can be observed.

Joint position error refers to the difference in the angle between the corrected posture and the natural posture of the head. This alteration is common in disorders associated with whiplash.

On the other hand, it has been shown that patients with whiplash and cervicogenic dizziness have balance problems when standing.

Neck pain is also one of the manifestations of these alterations, although its evaluation should always be carried out by a spine specialist.

How to diagnose cervicogenic dizziness?

When diagnosing cervical dizziness, it will be necessary to perform a comprehensive examination of the spine. In addition, a differential diagnosis will be carried out with other pathologies related to vestibular problems and visual problems.

Electronystagmography is the most widely used technique to evaluate patients with vertigo., so that you can observe how the eye movements work.

On the other hand, to detect the presence of dizziness of cervical origin, it is common to perform hearing tests

Additionally, a postural assessment can be performed using posturography, thus detecting possible proprioceptive alterations.

How to sleep with cervical dizziness?

Sleeping with cervical dizziness can be difficult, as many positions, such as rolling over, can cause discomfort. Some tips for sleeping well with this condition include:

  • Correct positionSleeping on your back can help keep your head and neck aligned with your spine. You can use a pillow that supports the natural curve of your neck.
  • Suitable pillowsOrthopedic or cervical pillows are specifically designed to properly support the neck and head, helping to maintain correct posture throughout the night.
  • Stretching before bed: Performing gentle exercises and stretching before bed can help reduce neck tension and relieve dizziness.
  • Regular sleep routine: Maintaining a regular routine can significantly improve the quality of your sleep.
  • Consult a specialistIf dizziness in the neck interferes with sleep, it is advisable to see a specialist who can offer treatment to relieve the symptoms.

Tips to prevent cervical dizziness

As we mentioned, neck dizziness is usually related to poor posture or repetitive muscle strain in the neck. Therefore, there are certain guidelines you can follow to prevent this type of dizziness from occurring. Some helpful tips to prevent it include:

  • Maintain good posture.
  • Take breaks throughout the day to stretch your neck and shoulders.
  • Sleep with a pillow that keeps your head and neck properly aligned.
  • Practice exercise regularly.
  • Avoid sudden movements.

Even following these recommendations often cannot prevent cervicogenic dizziness. At the first signs of symptoms, it is important to consult a spinal trauma specialist to prevent the problem from progressing and prevent possible complications.

Treatment for cervicogenic dizziness

The treatment with physiotherapy for cervicogenic dizziness It is one of the best options. In my case it is when I change my position, when I lie down it is horrible and even when I am lying down I cannot change my position because I feel like everything is coming down on me. What I can do? Thank you :'(

          • Hello Lis, this type of pathology requires a physical examination to be able to tell you exactly what is causing your dizziness.
Go to a spine specialist to find the cause and begin treatment.

          • Hi Claudia,
To ensure that these symptoms come from a cervical blockage, you should go to a consultation to be diagnosed, and from there, begin treatment.

They did an x-ray of my cervical spine and yes, I do have a rectification. They prescribed me Betagen and injected Miovit. I have been on the treatment for 5 days. My question please, how long will it take for me to start getting better?

          • Hello Luis,
Improvement may vary, but relief is usually seen within 1 to 2 weeks of treatment. However, cervical rectification may require physical therapy or specific exercises for lasting relief. Consult your doctor if symptoms persist.

          • As I understand you, 2 months ago they treated my otoliths and they told me to get kinesiotherapy for my cervical spine due to pain and dizziness. During the 2nd session, while I was there, I had vertigo for 1/2 hour and I couldn't even stand up. I had to be supported by my husband. I thought it was an attack... how can it be more? They give you massages. I feel like it's worse and today I'm scared. I don't want to do anything but I'm still feeling bad... I'm from Argentina.

          • Hello Veronica,
It's possible that kinesio tape triggered that episode of vertigo, especially if you have neck problems. If you feel that the therapy is making your symptoms worse, it's crucial to talk to your doctor. You may need to adjust the treatment or explore other options. Don't continue with kinesio tape without consulting first. Your health and well-being are the most important thing.

Unfortunately we do not have professionals in Panama. At first glance it seems that it is a case that must be investigated in detail to know what is happening. Spine cases are complex and require a lot of experience and analysis to make the diagnosis.
Many times it is necessary to consult with several professionals to find the cause of the problem.

Why did I suddenly notice dizziness that I had never had before and the tests went well, I just missed an MRI of the spine, but it's not vertigo, what happens is that my neck becomes unbalanced when I'm sitting and if I walk I go from one side to the other and I feel as if I were walking in the nines softly slow and lazy, it is strange that I have never felt

          • I also had some dizziness for a week when I was finishing with COVID. I was fine for a month and two weeks ago I was dizzy again and I had tests done and everything is fine. They sent me self 16 pills and nothing bad happens

The thing is that after the second week I started to get dizzy and I thought it was something specific and after the third week I got up and went to buy something and when I got there I started getting very dizzy and this dizziness lasted for a while. week and then I started to get better, but I noticed that when I moved or put away a lot that after a while it made me dizzy, I don't know if it's that being with anxiety and stress for so many years made my body get used to it and when I started to free myself from the tension something happened.

        • Hello Miguel,

I'm so sorry you're experiencing these symptoms. The dizziness, tingling, and numbness you mention could be related to cervical problems, such as nerve compression in the neck. These types of conditions can cause the symptoms you describe, so it's crucial that a specialist can make an accurate diagnosis and design an appropriate treatment plan to address both the symptoms and the underlying cause.

It's important that you continue to seek professional help to address your specific situation and prevent symptoms from worsening. In the meantime, try to reduce stress and anxiety, as these can intensify dizziness and other discomforts. It would be helpful to consider psychological support to manage these feelings and improve your quality of life.

        • Hi Miguel, I wanted to know if you've been diagnosed yet, because I suffer exactly the same as you. Even when I sleep, I get a sensation of electricity in my head, like waves. It's horrible. I was diagnosed with cervical rectification. I even wear a headset for motion sickness and I take Betaine. That only kept me well for two weeks, but the motion sickness hasn't gone away for four days. I hope you're feeling better now, because I understand. Being like this is horribly distressing and frustrating.

  17. Good morning, I'm Rufo
For approximately 2 years I have not been able to sleep on my back, or tilt my head back, because it makes me dizzy and dizzy; (when I am nervous it is much more accentuated); I have been to the physiotherapist many times; This difficulty forces me to postpone medical appointments in which I must be lying face up. I had a cerebrovascular Doppler and the veins are clear. The medications I take are diazepam stada 5 mg and sulpiride 50 mg. This setback is unliveable and I don't know if it has a solution. Thanks and regards.

    • Good morning Rufus,

Thank you for reading our trauma blog.

First of all, the case must be studied to assess that you do not suffer from neurological diseases such as Chiari, in addition to the cervical spine, as well as possible disc injuries or spinal instability. You can request an appointment with our trauma doctors to obtain a complete diagnosis.

  19. Hello!
I have had a problem for a long time now. I am diagnosed with generalized anxiety disorder and since I was a teenager I have cracked my back and neck a lot. Nowadays I am experiencing a lot of unusual dizziness, I feel like I am disconnected from reality, I feel unbalanced and it is difficult for me to do my daily things, all the time I want to lay my head down or sleep to calm this discomfort. It always hurts in the middle of my head like I have a bruise. I thought it could be something related to my spine and neck because maybe I damaged a nerve in my brain from thundering. I need help but I don't know exactly who to turn to... Could you tell me who I can treat myself to? Or are these symptoms due to anxiety itself?

  20. Hello good. A few days ago, I painted the ceiling in the room. I spent more than two hours with my neck bent backwards. That night I had no symptoms, but the next morning when I opened my eyes, everything was spinning. Since then I have had episodes of acute vertigo that last only a short time. The discomfort appears when lying down, never standing up, and occasionally when I swim in the pool, a sport that I have practiced for more than 35 years because I have lumbar disc herniations and swimming feels very good to me.

  23. For what it's worth, I'll tell you my experience: I'm on my way for five years with this... I started to improve when I finally went to an osteopath - a posturologist (not just any physio will do) - but not before having gone through countless specialists without head or tail... And after the fact I am going to a chiropractic to have my spine aligned…
It is important to correct posture and keep stress at bay. But above all, knowing how to manage EMOTIONS... because they accumulate there in the neck... In my case, anger and resentment... If you add to that many hours of work and/or study with your neck down, in front of the computer, and genetic predisposition... it is a ticking time bomb. There I am learning tools to manage those emotions. I read a lot about it, I do meditation, I put ice on the area, do some yoga, and I try to walk 30 to 45 minutes every day. Walking also helps heal the soul. No running because the cervical spine is affected.

Hello, one day I got dizzy while sitting and since then I have had dizziness, imbalance, a heavy head and I feel like it weighs me down and hurts at times, lights and sounds have also bothered me. I feel like I'm falling when I walk. I went to the ENT doctor and he told me it could be migraines, but I went to the neurologist and they did an MRI on my brain and an x-ray on my cervical spine and everything came out fine. He told me it could be a contracture in my cervical spine and he sent me for cervical physiotherapy. . My fear is that this dizziness will not completely go away after physical therapy. I also suffer from scoliosis. I don't know which other specialists to go to. I have taken all types of medication sulpiride dizinel and sibelium and various anxiolytics and nothing. The neurologist gave me things for the contracture such as muscle relaxants and anti-inflammatories but I don't know how the dizziness is going at all. I don't know what other specialists I can go to. If anyone else is in my situation, please contact me. It's a horror to live like this, thank you.

  37. Hello, I started with constant dizziness and instability 15 days after having Covid in June 2022. I have been through 4 ENT specialists, a specialized center for vertigo and dizziness, a Neurologist, a Cardiologist, I had a cranial magnetic resonance imaging, various studies of the ears and everything goes well. They sent me to a psychiatrist and he didn't find anything relevant either.
I went to the chiropractor because of severe back pain and I asked him if he could check my cervical spine and when he did so he found a cervical rectification, that is, my spine is very straight and that is apparently causing the dizziness.
I am being treated by a chiuropractor and I had a notable improvement, but then the dizziness and vertigo return again.
Everything points to a cervical problem since everything else goes well

I can't turn around suddenly, it gives me imbalance, I can't lie on my back, I sleep with 3 or more pillows, it's exasperating, days, weeks, months, so I did all the possible studies and everything turns out fine if I have a rectification. chronic cervical vertigo, I can't stand without holding myself up or walk alone, bathe, etc. It's horrible, I did kinesio physio but I see that nothing has the definitive solution, I take betahistine, even so, everything revolves around sometimes... the heat, the cold, the smoke, everything affects stress. Many people sleep little during the confinement...fear is present whenever they tell us not to be afraid when the body cannot support it, it is difficult but not impossible, I only wish that all of us who went through this can get ahead 🙌🏽

other times when I lower my head I get drowsy or my eyes turn red

My neck doesn't hurt but it gets hot sometimes and this only happens when I'm standing because if I'm lying down all day it doesn't hurt at all.
The pain in my head appears mainly if I'm stressed in front of the computer, but sometimes even that doesn't bother me.
other times when I have my head in certain positions I also feel strange things like I'm disconnected for 1 second and that already makes me think that it is because of the neck that traps a vein but the symptoms are so many that I don't know what to think because I have had several tests and they say it's psychological.

  53. Hello. I quickly comment on my situation.
I have had GAD problems for a few years now, and on three occasions (in 2016, 2018-2019 and now 2024) it has happened to me that it gives me that feeling of vertigo. Since around March 2024, I had already been experiencing very high stress and panic attacks for no apparent reason. I spent almost all my time lying in bed with my laptop working, poor posture and a lot of stress. I started to feel strange things in my neck and shoulder, on the side of my ear (on my jaw almost at the entrance to my ear), and then in my head. Sometimes I would get a headache in the neck area. As the last few months passed, I began to feel a feeling of depersonalization or like being in a dream or lethargy. Just on June 14, 2024 in the morning, I was woken up by the feeling of vertigo (which only lasted a second or two). I was like this for two days half bad, but I could levitate myself from bed, walk and do my things (halfway), if I bent down, raised my head or lay down I got dizzy; not much but like an imbalance. I was doing massages and hot-cold compresses with ice, and head and neck movement exercises; And I think that helped me a lot. Little by little the dizziness began to subside but not that "drunk" feeling. Another thing to note is that I did anger with a client just about 8-10 days before the vertigo crisis.

Although as of today (July 10, 2024), it has not gone away 100%, I can say that I feel 80-90% better. These days I have noticed that the muscle behind my neck on the left side feels hard, a kind of pull or something that causes me a "tingle" and sometimes I feel it reflected up to the left eye (inside) and in the left buttock as well. It is at this moment when I get that feeling of imbalance (not dizziness as such or vertigo), but unstable, and when I massage myself and half release that "sensation" from my neck the entire symptom disappears. When I stretch my neck, massage or put a lot of pressure on that muscle, I can feel something contracted or tense there, with a desire that it should "thunder" to release the tension. Could it be something from the cervical tension, the muscle or something similar? Greetings from Mexico and thanks for reading.

Until they prescribed meniex 16. But I can't not take it to avoid it because it's like daily. I need if you can guide me, I even saw a neurosurgeon, who told me that surgery is not necessary yet.

Before the accident I was already dizzy and the doctor diagnosed me with lordosis rectification. Now after the accident I am much more dizzy. I don't know if the blow can aggravate the problem I already had before?

    • Hello Miriam,
Yes, the blow may have aggravated the problem of lordosis rectification and worsened the dizziness you already had. I recommend that you consult your doctor for a more in-depth evaluation and to rule out possible additional complications.

I'm sorry you're going through this. The symptoms you describe, such as dizziness, neck pain, pressure in the head, and nausea, could be related to a number of conditions, such as neck problems or even migraines. It's critical that you seek urgent medical attention for a full evaluation. A doctor might recommend tests such as an MRI or a consultation with a neurologist to rule out more serious problems and find the right treatment.

  60. Hello, I'm from Colombia and 5 years ago I was diagnosed with neurocardiogenic syncope, since then I have many health problems and after a while I feel bad all the time because I have vertigo, my head hurts so much sometimes I can barely walk because I feel no strength in my legs and if I stand up it's like I have no balance, I get nauseous and want to vomit, that discomfort sometimes doesn't let me sleep because if I move my head even a little bit the sensation is horrible, and I have a lot of back pain especially on the top of my neck, it lasts for many days and no matter how many medications I take it doesn't go away, I would like some help please to know what it could be that I have and what specialist I should go to that could help me. I also have lumbar and cervical scoliosis, but the doctors tell me that it is mild and not important, and all they tell me is that everything that happens to me is because I suffer from syncope, but I am not so sure about that because I hardly ever get syncope and I still have the rest of the ailments all the time.

    • Hello Nereida,
It's important to see a neurologist and an ear, nose, and throat specialist (otolaryngologist) to rule out neurological or balance problems (such as vertigo). It would also be helpful to check your spine for possible problems due to scoliosis.

I need to specify well that I have written regimens, sommeil ok, pas de signes avants courreurs ni prevention du problème, parfois je pas de rares nuits calmes.

Certain symptoms that you experience, such as recurring vertiges and ocular tension, are also linked to neurological problems. L'évaluation par un neurologue pourrait être usefule pour confirmer ou écarter si les vertiges ont une cause neurologique.

Concerning spinal colon problems, although MRI does not reveal serious changes, it is possible that cervical problems influence vertiges. An integrated approche, combining a spécialisée physiothérapie and the specific treatments for vertiges, pourrait être bénéfique.

    Findings:
    Restification of cervical lordosis.
    Vertebral bodies of preserved height.
    Signs of disc dehydration.
At c4-c5 a diffuse posterior disco-osteophytic protrusion is seen, more prominent at the right central and paracentral level where it flattens the right anterior contour of the spinal cord.
At C5-C6 a disc protrusion is seen that partially obliterates the anterior subarachnoid space and extends to both foramina, causing biforaminal stenosis, greater on the right side.
Normal marrow thickness and signal intensity.
    Well-positioned cerebellar tonsils.
In summary, MRI of the cervical spine with lordosis restification and prominent C4-C5 and C5-C6 disc protrusions.

    • Hello Estefania,

The findings on your MRI may be related to your dizziness, especially if there is nerve compression or vascular involvement in the cervical area.

To determine the most appropriate treatment, a detailed evaluation by a specialist is essential. Depending on the severity of the compression and your symptoms, options may include specialized physical therapy or other medical treatments.

    • Hello Elena,

We're very sorry you're experiencing these symptoms. Neck pain and dizziness may be related, as muscle tension in the neck can affect balance and cause headaches, such as migraines.

Given the complexity of your symptoms, it is important that you consult a specialist for a thorough evaluation and to rule out any other possible causes. The appropriate treatment will depend on the precise diagnosis, and additional testing may be necessary.

Concerning your question, it is possible that certain conditions affect the cervical colon and are not visible on IRM or on radiographs, especially on functional problems or on articular movements that are not visible on images.

Our recommendations are to continue to consult a spécialiste de la colonne vertebrae, car d'autres approches diagnostiques peuvent être nécessaires, comme une évaluation clinique plus détaillée ou des tests fonctionnels. Furthermore, if vertiges persist, it is useful to exclude other possible causes that are not directly linked to the cervical colon.

Si vous le souhaitez, dans notre centre, nous avons des professionnels qui pourraient vous aider à évaluer votre cas de manière plus personnalisée.

Nous vous adressons nos salutations les most cordiales et tout notre soutien.

We're sorry you're experiencing these symptoms. Dizziness can be due to a variety of causes and, in some cases, can be related to cervical problems. If you haven't already, we recommend seeing a spinal orthopedic surgeon to evaluate whether your condition may be related to this area.

  78. Good morning. I would like you to please help me. What happens is that I have an intense pain in the back of my head, especially on the left side. My left ear is blocked. The left side hurts and I start to get dizzy. When the noise in my ear increases, as if it were wind, the dizziness starts to get more intense and I end up vomiting. However, this increase in noise in my ear appears when I get nervous or complain and it increases in a matter of 15 to 20 minutes at most until I get intense dizziness and I start to vomit. What do you advise me to do because I have already done all the blood tests and ultrasounds and I have absolutely nothing. The doctor tells me that everything is normal. But with your experience, maybe you can tell me if any patient has presented with the same symptoms and what treatment I can do or where I can go to you or what type of analysis or study I should do. Please, I would ask you to help me.

In the meantime, try to avoid situations that trigger symptoms and don't hesitate to go to the emergency room if dizziness or vomiting worsens.
